CONTRACT OF ADHESION AND EXEMPTION OF RESPONSIBILITY OF HEALTH AND PHYSICAL CONDITION
I, have voluntarily enrolled in a Training and Fitness Program offered through MY CARDIO SOUL and ALEJANDRA TROCONIS. I acknowledge that the program may involve strenuous physical activity including, but not limited to, strength and muscle resistance training, cardiovascular conditioning, and various other physical conditioning activities. I hereby affirm that I am in good physical condition and do not suffer from any known disability or condition that may prevent or limit my participation in this exercise program. I have been informed that anyone should obtain a medical examination before beginning an exercise and / or exercise program, or initiating a substantial change in the amount of regular physical activity performed. If I have decided not to obtain the consent of a physician before beginning this fitness program, I agree that I do so at my own risk. I understand that it is my sole responsibility to participate in exercises that are appropriate for the current state of my health. If I have any questions or concerns about whether or not a particular activity is appropriate for my current health condition, I understand that it is my responsibility to ask my doctor if this activity is appropriate before participating in that activity. I agree not to hold MY CARDIO SOUL or ALEJANDRA TROCONIS responsible for the actions or omissions of the other program participants.
I have been informed, understand, and am aware that strength, flexibility, and aerobic exercise, including the use of equipment, are potentially dangerous. I have also been informed, understand and am aware that physical activities pose a risk of injury and that I am voluntarily participating in these activities, using equipment with full knowledge, understanding and appreciation of the risks involved.
I hereby waive and release MY CARDIO SOUL, ALEJANDRA TROCONIS and their successors and assigns, from each and every claim, cost, liability and expense of any injury, loss or damage, whether known, anticipated or unanticipated to arise from my participation and voluntary enrollment in your Fitness and Training Program.
I ACKNOWLEDGE THAT I HAVE COMPLETELY READ AND UNDERSTAND THIS FORM IN ITS ENTIRETY AND I KNOW IT CONTAINS A RELEASE OF LIABILITY.
